Students are responsible for notifying the Registrar's Office every term of their present address and telephone number regardless of whether or not it has changed. Please use this form whenever you begin a new rotation. If any changes occur to any of your address/telephone information after completing this form, please resubmit this form to the Registrar's Office immediately. If you enter a valid College email address, you'll receive a copy of this form via email.
